This new format is being used to provide all Sokudo with access to information about both Injury and Non-Injury incidents and use this information as a sort of "lessons learned" to help prevent similar incidents in the future.

 

This new format includes the following sections:

  • Log of Work-Related Injuries & Illnesses (OSHA Form 300).

  • Summary of Work-Related Injuries & Illnesses (OSHA Form 300A).

  • Injuries & Illnesses Incident Report and Investigation Results (OSHA Form 301).

    • These Incident Reports include links to:

      • Accident Report for Workers Comp Claim.

      • Incident Investigation Report (includes Root Cause, Contributing Causes, and Recommended Corrective Actions).

    • Note: These documents have been "sanitized"; all personal, confidential, or sensitive information about the injured employee has been removed. This includes information such as: Social Security Number, Home Address/Phone, and Date of Birth.
