Safety Incident/Accident Reporting Information Page

How to Report a Safety Incident

 The Affected Employee is responsible for reporting the incident to the Safety Manager, using the Online Accident Report  (OAR).

All safety incidents, injury and non-injury, must be reported immediately. All Sokudo Employees must follow these steps for all safety incidents.

Attend to any urgent medical needs (see Medical Treatment section below).

Notify your Manager, Supervisor, Lead,  or Coordinator of the incident during or immediately after seeking treatment (via phone, email, pager, or in person).

Affected Employee Complete and Submit the Online Accident Report (OAR) via either the Intranet or Internet site.

                          Or

  • Call the Safety Manager to report the incident.

If on a customer site, follow the customer’s procedure for notifying their personnel of the incident, and complete their forms as required. If you are not familiar with this procedure, contact your sponsor at the customer facility. Immediately after incident.

Department or Regional Manager Initiate incident investigation using the Accident Investigation Kit maintained by each Department or Regional Manager. Ensure that evidence collected is delivered to the Safety Manager. Immediately after incident.

Safety Manager

A.  For injury incidents, submit Online Workers Comp Claim Form to insurance carrier.

A.      W/I 2 hrs receipt of OAR

B.   Contact Employee and provide: (a) The Worker’s Comp (WC) Claim Number and (b) Link to WC  information for injured employees.

B.      Immediately after receiving WC Claim #.

C.   Review evidence and conduct interviews with Employee, Witnesses, and Department or Regional Manager as appropriate.

C.      Throughout investigation.

D.   Contact treating physician and obtain diagnosis and treatment records.

      Manage WC claim throughout the treatment process by communication with the insurance carrier, treating physician,  Employee, and  Department or Regional Manager. Keep the Department or Regional Manager informed on the progress of the Incident Investigation.

D.      Throughout investigation.

E.    If work restrictions are assigned by physician, provide Department or Regional Manager with link to the Return To Work Program (RTW) information page and RTW Application. Discuss this Program with the Manager to determine if it is appropriate for the employee's work situation.

E.      Throughout investigation.

F.   Following investigation by the Safety Manager and the WC insurance carrier, create a full written report on findings and recommendations. This report will be made to the Department or Regional Manager.

1.   Depending on the nature of your injury you may need to seek medical attention.  If not sure, go ahead and seek medical care.

2.  You may see your personal physician or visit a local clinic. Make sure you advise the physician that this is a work-related injury.

3.   Give the clinic our Workers Compensation information .

The Safety Manager will also need the contact numbers for your doctor.

4.   All Doctors and clinic forms/notes must be forwarded to the Sokudo Safety Manager (Fax: 562-420-6762).
